Hours per week:  Full time - 35 hours per week
Salary:  £22,000pa rising to £23,000pa after probation
Location:  North East England

Job Description:

To work with families and professionals at Great North Children’s Hospital, ensuring that CYP and their families have the appropriate information and support available to them on discharge. To work with community based professionals and partner organisations in ensuring a smooth transition from hospital into community services and back into education, working across Newcastle and surrounding areas.
